ISI Testing Forms

These are the ISI testing forms in PDF format to be downloaded, printed and used by ISI Professional members to test skaters through the ISI Ice Skating Program. A current ISI individual skater membership is required for all skaters who take an ISI test and register that test with the ISI national office.

All Level 8 and 9 tests must be taken in a District Testing Program or by video test submission to the national office.  All Level 10 tests and Pair 6-9 tests must be taken at an ISI national event as outlined in the current ISI Handbook.
